Fentanyl drove a steep rise in Dallas-area overdose deaths through 2023, with early signs of decline in 2024 — against a backdrop of unusually limited state harm-reduction policy.
Dallas County recorded 282 fentanyl-related deaths in 2023, with fentanyl involved in 79.8% of all overdose deaths in the county, according to Dallas County and Texas DSHS data. The county's overdose death rate surged roughly 81% between 2019 and 2023. Encouragingly, county figures show fentanyl deaths falling sharply in 2024 as prevention and naloxone efforts expanded.
As across the country, illicitly manufactured fentanyl — not prescription opioids — drives the deaths, increasingly in the form of counterfeit pills ("M30s") and mixed into the stimulant supply. People who believe they are taking a real pill or using cocaine or methamphetamine face opioid-overdose risk, which is why naloxone matters for everyone.
Dallas faces this crisis under some of the most restrictive harm-reduction laws in the country. Texas has not legalized syringe services statewide, and fentanyl test strips remain classified as drug paraphernalia — meaning a basic, proven overdose-prevention tool is not legally available the way it is in many states. Parkland Health
Parkland Health is Dallas County's public safety-net hospital and a Level I trauma center. Texas's overdose Good Samaritan provision was created by HB 1694 (the "Jessica Sosa Act"), effective September 1, 2021. It establishes a defense to prosecution — not blanket immunity — for possession of small amounts of a controlled substance or paraphernalia for a person who in good faith calls for emergency medical help for a suspected overdose, and stays at the scene.
Crucially, the protection is narrow. It does not apply if the caller has a prior felony conviction, if they have already used the Good Samaritan defense or sought overdose help within the past 18 months, or if a peace officer was already in the process of arresting or searching them. Public-health advocates have criticized these carve-outs for excluding many of the people most likely to witness an overdose.

Texas PMP (prescription monitoring)
Texas's prescription drug monitoring program is the Texas PMP, administered by the Texas State Board of Pharmacy. Dispensed Schedule II–V controlled substances are logged, and prescribers and pharmacists are generally required to check the PMP before prescribing or dispensing opioids and other controlled drugs. Patients may request their own record from the Board of Pharmacy.

Will I get arrested if I call 911 for someone overdosing in Dallas?
It depends — Texas's protection is limited. Under HB 1694 (the Jessica Sosa Act), a person who in good faith calls for medical help for an overdose and stays at the scene has a defense to prosecution for small-amount drug or paraphernalia possession. But the protection does not apply if you have a prior felony conviction, if you sought overdose help (or used the defense) in the past 18 months, or if an officer was already arresting/searching you. Despite the limits, calling 911 is still the right and safest choice — stay with the person and give naloxone if you have it.
Does Texas Medicaid cover rehab in Dallas?
For those who qualify, yes — but Texas has the highest uninsured rate in the country and did not expand Medicaid, so many low-income adults (especially those without dependent children) do not qualify at all. Texas Medicaid (STAR managed care) and CHIP cover substance-use treatment and medication for opioid use disorder for eligible members.
