The program uses the SAMHSA evidence-based ACT model adapted for older adults, delivering more than 75% of services directly in the community — in shelters, on the street, in supportive housing, or in clients' homes. The multidisciplinary team provides geriatric psychiatric evaluation and medication management, substance use treatment for co-occurring conditions, skilled nursing, psychosocial rehabilitation, housing placement and stability support, help with benefits (SSI/SSDI, Medi-Cal, Medicare), and assistance with daily living.
