Specialized Geriatric Behavioral Health Services
Senior IMPACT employs the SAMHSA evidence-based Assertive Community Treatment model, adapted to address the unique clinical and social needs of older adults. The program's multidisciplinary teams deliver more than 75 percent of services directly in the community, meeting clients wherever they are — in shelters, on the street, in supportive housing, or in their own homes. ACT fidelity is evaluated annually by an external consultant to ensure the highest quality of care.
The comprehensive service array includes specialty behavioral health assessments and psychosocial rehabilitation, geriatric psychiatric evaluation and medication management, substance use treatment for co-occurring conditions, skilled nursing care addressing the intersection of physical and mental health in aging populations, housing placement and ongoing housing stability support, assistance with benefits and entitlements (SSI/SSDI, Medi-Cal, Medicare), care coordination with medical providers and aging services agencies, and assistance with activities of daily living such as personal hygiene, nutrition, and mobility.
Countywide Reach for Older Adults
Unlike some ACT programs that serve specific geographic catchment areas, Senior IMPACT serves eligible older adults throughout all of San Diego County, recognizing that geriatric homelessness is not confined to the downtown core. This countywide mandate ensures that seniors in suburban, rural, and exurban areas of the county can access the same intensive services available in the urban center.
Access and Eligibility
Eligible individuals must be 60 years of age or older, reside in San Diego County, have a serious mental illness with functional impairments, experience homelessness or be at risk of homelessness, and be deemed to require ACT-level services based on the LOCUS assessment. The program accepts voluntary participation as well as individuals with conservators willing to sign consent for services. Senior IMPACT accepts Medi-Cal, Medicare, and uninsured clients. The program is funded by the County of San Diego HHSA Behavioral Health Services and operates Monday through Friday with evening and weekend outreach, with after-hours support available at (619) 342-9099.
