The overdose picture in San Diego County
Fentanyl drove San Diego County overdose deaths to roughly 800 a year — but 2023 brought the first decline, alongside one of the country's more supportive harm-reduction policy environments.
San Diego County averaged about 800 fentanyl-related deaths a year in 2021 and 2022, and in 2023 recorded the first decrease since the county began tracking fentanyl deaths, according to the County of San Diego HHSA. As a major border and port region, San Diego sits at the front line of the illicit-fentanyl supply.
As elsewhere, illicitly manufactured fentanyl drives the deaths — increasingly in counterfeit pills and mixed into the cocaine and methamphetamine supply, so people who do not knowingly use opioids still face overdose risk. Unlike states such as Texas, California has made the basic prevention tools legal and available: naloxone and fentanyl test strips are distributed widely.
In fall 2023 the county launched its Overdose Surveillance and Response (OSAR) program — a collaboration of Public Health, Behavioral Health, the Medical Examiner, and EMS that does street-level outreach to distribute naloxone and fentanyl test strips and link people to care. This data-driven, harm-reduction-forward response is part of why deaths began falling.
What the local response looks like
The county's OSAR program and California's Naloxone Distribution Project provide free naloxone and fentanyl test strips — both fully legal in California.
California expanded Medi-Cal, and San Diego is a Drug Medi-Cal Organized Delivery System (DMC-ODS) county, so Medi-Cal members can access the full ASAM continuum of substance-use treatment.
The San Diego Access & Crisis Line (888-724-7240) connects residents to crisis support and treatment 24/7 in more than 200 languages — a single front door regardless of insurance.
For San Diego residents and families searching today: naloxone and test strips are free and legal, Medi-Cal covers treatment for those who qualify, California law protects you for calling 911, and one phone number reaches help any hour. The sections below map the local system.
San Diego crisis and helpline numbers
If you are in immediate danger, call 911. Otherwise, the following are free, confidential, and available 24/7 unless noted.
Where San Diego's addiction care actually happens
San Diego's system runs from the county Access & Crisis Line and Behavioral Health Services through academic and safety-net hospitals, the VA, large nonprofit SUD providers, and federally qualified health centers.
San Diego Access & Crisis Line / County BHS
The San Diego Access & Crisis Line (ACL), operated for County Behavioral Health Services, answers 888-724-7240 24/7 in more than 200 languages. It provides crisis support and connects residents to the county's Drug Medi-Cal Organized Delivery System (DMC-ODS) network of detox, residential, and outpatient SUD treatment — the best first call for most San Diegans, insured or not.
UC San Diego Health
UC San Diego Health is the region's academic medical center, providing emergency care, addiction medicine, and co-occurring psychiatric treatment. Its emergency departments are common points of contact after an overdose, with buprenorphine initiation and warm hand-offs to outpatient care.
McAlister Institute
McAlister Institute is one of San Diego County's largest nonprofit substance-use treatment providers, operating detox, residential, outpatient, perinatal, and youth programs across the county — many funded through Drug Medi-Cal and county contracts, making them accessible to Medi-Cal members and low-income residents.
VA San Diego Healthcare System
The VA San Diego Healthcare System provides substance-use treatment for enrolled veterans, including medication for opioid use disorder and care for co-occurring PTSD and depression. Veterans in crisis can dial 988 then press 1.
Family Health Centers of San Diego (FQHC)
Family Health Centers of San Diego is one of the region's largest federally qualified health center networks, offering sliding-scale integrated primary care, behavioral health, buprenorphine prescribing, and harm-reduction services. Find this and other centers via the HRSA health center finder; FQHCs cannot turn patients away for inability to pay.
County OSAR / harm-reduction outreach
The County's Overdose Surveillance and Response (OSAR) program conducts street-level outreach in the hardest-hit areas, distributing free naloxone and fentanyl test strips and connecting people to treatment and community resources — a model made possible by California's supportive harm-reduction laws.
What rehab costs in San Diego — and how Medi-Cal helps
California expanded Medi-Cal, so far more low-income San Diegans qualify for free SUD treatment than in non-expansion states — a major access advantage.
California expanded Medi-Cal under the ACA, covering adults up to 138% of the federal poverty level, and in recent years extended full-scope Medi-Cal to income-eligible residents regardless of immigration status. For San Diegans, that means a far wider safety net than in non-expansion states — a decisive difference for affordability of treatment.
San Diego County participates in the Drug Medi-Cal Organized Delivery System (DMC-ODS), which covers the full ASAM continuum of substance-use treatment for Medi-Cal members: outpatient, intensive outpatient, residential, withdrawal management (detox), and all medications for opioid use disorder. Access is coordinated through the county and the Access & Crisis Line (888-724-7240).
Commercial insurance is widely held in San Diego — major payers include Anthem Blue Cross of California, Blue Shield of California, Kaiser Permanente, UnitedHealthcare, Cigna, and Aetna. Under the federal Mental Health Parity and Addiction Equity Act and California parity law, plans must cover substance-use treatment on par with medical care. Confirm in-network status before admission.
For uninsured residents, San Diego's options are relatively strong: the Access & Crisis Line (888-724-7240) for navigation, McAlister Institute and other county DMC-ODS providers for Medi-Cal and county-funded treatment, FQHCs like Family Health Centers for sliding-scale care, and OSAR outreach. Self-pay rates at private residential programs in San Diego typically run $15,000–$40,000+ for a 30-day inpatient stay (San Diego skews higher-end) — ask each program about sliding-scale or scholarship beds.
California's Good Samaritan law and naloxone access
Calling 911 for an overdose in San Diego is protected under California Health & Safety Code § 11376.5 — a relatively broad immunity.

- Short title
- California 911 Good Samaritan overdose immunity
- Enacted by
- California Legislature (AB 472, 2012)
- Effective
- January 1, 2013
California's overdose Good Samaritan statute is codified at Cal. Health & Safety Code § 11376.5. It provides that it is not a crime to be under the influence of, or to possess for personal use, a controlled substance or paraphernalia when a person in good faith seeks medical assistance for someone experiencing a drug-related overdose and does not obstruct medical or law-enforcement personnel. The same protection covers the person who overdoses and needs help.
Under California law, naloxone is available without a personal prescription through a statewide standing order and over the counter, and the California Naloxone Distribution Project supplies free naloxone to community organizations. Fentanyl test strips are legal in California and distributed by county and community harm-reduction programs.
What the immunity does NOT cover
- Selling, providing, giving, or exchanging controlled substances
- Forcing or coercing another person to take drugs
- Outstanding warrants or unrelated offenses
- Obstructing medical or law-enforcement personnel at the scene
In plain terms: if you call 911 for an overdose in San Diego, you cannot be charged for being under the influence or for personal-use possession discovered because you sought help — California's protection is broader than many states'. The safest action is always to call 911, stay, and give naloxone. The full statute is on California Legislative Information.
CURES (California prescription monitoring)
California's prescription drug monitoring program is CURES (Controlled Substance Utilization Review and Evaluation System), administered by the California Department of Justice. Dispensed Schedule II–IV controlled substances are logged, and prescribers are generally required to consult CURES before prescribing. Patients may request their own record from the DOJ.
San Diego County drug courts
Non-violent defendants with substance-use disorders in San Diego may be eligible for San Diego County drug courts and collaborative courts, treatment-supervised diversion run through the Superior Court. Successful completion can reduce or dismiss underlying charges; eligibility is determined through the court and defense counsel.

Unlike many states, California makes the core harm-reduction tools fully legal and free. The County's Overdose Surveillance and Response (OSAR) program and community partners distribute free naloxone and fentanyl test strips through street outreach, and the California Naloxone Distribution Project supplies naloxone to organizations statewide. Naloxone is also available without a prescription at any San Diego pharmacy and over the counter. Call the Access & Crisis Line at 888-724-7240 for current free-supply sites.
Getting naloxone for free
Beyond pharmacies, free naloxone comes through the California Naloxone Distribution Project, County OSAR outreach, FQHCs like Family Health Centers of San Diego, and many hospital emergency departments after an overdose visit. The Access & Crisis Line (888-724-7240) can direct you to current sources.
Mutual-aid recovery networks
The San Diego AA Central Office maintains a searchable directory of hundreds of weekly Alcoholics Anonymous meetings across the county — in-person, hybrid, and special-focus formats. Narcotics Anonymous meetings are listed through San Diego–Imperial Counties NA. SMART Recovery and Refuge Recovery meetings also run weekly in San Diego.
Peer & recovery community support
California has a Medi-Cal-reimbursable peer-support specialist benefit, and certified peer specialists are embedded in many San Diego treatment programs, hospital EDs, and recovery community organizations. Ask any provider or call the Access & Crisis Line (888-724-7240) about connecting with a peer specialist.

Frequently Asked Questions
Will I get arrested if I call 911 for someone overdosing in San Diego?
No — California's protection is relatively broad. Under Health & Safety Code § 11376.5, it is not a crime to be under the influence of or to possess for personal use a controlled substance or paraphernalia when you in good faith seek medical help for an overdose and do not obstruct responders — and the same protection covers the overdose victim. It does not cover selling/providing drugs, forcing someone to use, outstanding warrants, or obstructing responders. Always call 911, stay with the person, and give naloxone if you have it.
Does Medi-Cal cover rehab in San Diego?
Yes — and broadly, because California expanded Medi-Cal. San Diego is a Drug Medi-Cal Organized Delivery System (DMC-ODS) county, so Medi-Cal members can access the full ASAM continuum: outpatient, intensive outpatient, residential, withdrawal management (detox), and all medications for opioid use disorder. California has also extended full-scope Medi-Cal to income-eligible residents regardless of immigration status. Start with the Access & Crisis Line at 888-724-7240 or a county DMC-ODS provider like McAlister Institute.
Where can I get free naloxone and fentanyl test strips in San Diego?
Both are legal and free in California. The County's OSAR program distributes free naloxone and fentanyl test strips through street outreach, and the California Naloxone Distribution Project supplies naloxone to community organizations. Naloxone is also available without a prescription at any San Diego pharmacy and over the counter, and FQHCs and many hospital EDs provide it. Call the Access & Crisis Line at 888-724-7240 for current free-supply sites.
How much does rehab in San Diego cost without insurance?
It depends on the setting, but California's Medi-Cal expansion means more residents qualify for free treatment than in most states. If uninsured, call the Access & Crisis Line (888-724-7240) for navigation, or contact a county DMC-ODS provider like McAlister Institute or an FQHC like Family Health Centers of San Diego for Medi-Cal or sliding-scale care. Self-pay rates at private residential programs in San Diego typically run $15,000–$40,000+ for a 30-day inpatient stay (the area skews higher-end) — ask each program directly about sliding-scale or scholarship beds.
Can I get medication for opioid use disorder (buprenorphine or methadone) in San Diego?
Yes. Buprenorphine (Suboxone) is prescribed at many San Diego outpatient clinics, FQHCs, and emergency departments — UC San Diego and other EDs initiate it after overdoses. Methadone is dispensed only at federally certified opioid treatment programs (OTPs), as required by federal law (42 CFR Part 8), several of which operate across the county. Medi-Cal (DMC-ODS) and most commercial plans cover medication for opioid use disorder. If cost is the barrier, an FQHC or the Access & Crisis Line (888-724-7240) is the best place to start.
What is the fastest way to get help in San Diego right now?
Call the San Diego Access & Crisis Line at 888-724-7240 any hour — it offers crisis support and treatment connection in 200+ languages — or dial 988 for the Suicide & Crisis Lifeline.
